This form should be read verbatim to the client. It ensures that the client understands that by signing this form that they understand that they have
the right to plead not guilty to any offense of which they have been charged and that if they choose to plead not guilty they have the right to a speedy and public trial by jury; that at trial they have the right to the assistance of counsel, the right to confront and cross-examine witnesses testifying against them, the right to use the subpoena process of the court to compel the production of any evidence, the right not to be compelled to testify or incriminate themselves, and the right to have a jury impose any fine in excess of $50.00. It further explains that if they choose to have a trial and are convicted, they have the right to appeal that conviction to the Court of Criminal Appeals of the State of Tennessee and the right to the assistance of counsel in obtaining a review of their conviction by the appellate court and by signing this form, they are waiving all of those rights.  By signing this form, the criminal defendant also acknowledges that they have discussed the case with their criminal defense lawyer, understand the criminal charges against them as well as any defenses that could be raised if their case went to trial.  The waiver also ensures that they understand that in accepting
 
this plea of guilty, the court may ask  them questions and require that they answer under oath, on the record, with the assistance of their attorney, and that they are not truthful in their responses, their answers may later be used against them in a prosecution for perjury or the making of a false statement.
